Policies
We kindly ask for a credit card on file which may be used to pay any balance that may be due on your bill, including late fees.
A $25 late fee will be charged for each occurrence of appointment cancellations made less than 24 hours in advance. We will endeavor to contact you before processing this charge.
Intake paperwork will include your signature authorizing Align & Thrive Chiropractic LLC to keep your signature and your credit card information securely on-file. You permit Align & Thrive Chiropractic LLC to charge your credit card for any due outstanding balances or late fees.
If the credit card that you give at that time changes, expires, or is denied for any reason, you agree to immediately give Align & Thrive Chiropractic LLC a new, valid credit card which you will allow them to charge over the telephone. You agree that the new card may be used with the same authorization as the original card presented.
